    exactly what it says...sister-son = nephew = sister's son

    Maeglin = Aredhel's son = Turgon's sister-son.

    Éowyn = Théodwyn's daughter = Théoden's sister-daughter

    I suspect that a student of old Norse or Anglo-Saxon might say that these languages contained such usage.
